6Agreements and e-signature

I have read and agree to the Registration, Waiver & Medical Release (PDF), including the assumption of risk, the release of liability for Smith County Soccer and YWAM Lindale, and the medical treatment authorization. I have read the CDC HEADS UP concussion information and will review it with my child. A player with a suspected concussion sits out and returns only with written clearance from a licensed health care professional. My child and I will follow the Code of Conduct: respect for coaches, players, referees and YWAM Lindale's property and rules. Payment terms: $80/month per player per program by autopay. To cancel, give written notice before the next billing date (notice period TBD). No refunds for missed sessions or weather cancellations; make-ups when possible.
